Community Entry

The Health Guardian Program

Health Guardians are EHS's trusted presence in the community. They register families, run an initial health assessment, share information about EHS services, and hand every case to the Help Desk for coordination.

A Health Guardian does not: diagnose or treat any condition, prescribe medicines, recommend a specific treatment or specialist, or interpret lab results. Their role is connection, not clinical decision-making.

What a Health Guardian does

Runs an initial health assessment
Educates families about EHS services

Operating Model · OM-001

One patient. One record. Lifetime care.

Every patient is registered with a unique EHS Patient ID and a dedicated file — registration form, consultation notes, prescriptions, lab and imaging reports, referral forms, and follow-up notes, all kept together and indexed in the EHS Master Register.

We may not run on complex software today, but every record is disciplined, organized, and handled by authorized personnel only — because continuity of care depends on it.
